In readiness for your appointment, we request that you print out and fill our patient registration form.
After you make your appointment, you will receive in the mail information for Digichart. You will need to access this website and fill out your online history prior to your appointment. If you do not complete the online health history prior to your appointment, we may need to reschedule your appointment in order for us to provide you the most complete care. If you have any difficulties with this process or do not receive your packet, please call us so we can assist you.
We strive to run our schedule on time. This requires a commitment on our part as well as yours to adhere to your appointment times. Out of respect for the patients that follow you in our office schedule, you may be asked to reschedule if you arrive late. We request that for annual exams, gynecology appointments and new patient appointments you arrive 15 minutes before your appointment time to allow for the registration process.
We will bill insurance as appropriate for the services you receive. If your insurance requires a co-payment, this will be collected at the time of your appointment. If you have questions about billing, please feel free to contact our billing staff.
When emergencies, deliveries, and unforeseen delays take us out of the office, we will try to minimize your inconvenience by notifying you. If needed, appointments will be rescheduled. Siblings are welcome at prenatal visits. However, for your child’s safety and the consideration for other patients, we discourage unattended young children in our office waiting room.
Cancellation Policy-we require 48-hour notice of cancellaitons. We reserve the right to charge for late cancellations and no-show appointments.
